Enteral Feeding Orders Not Followed and Supplies Not Properly Labeled
Summary
The facility failed to provide necessary care and services related to GT enteral feeding for four residents. The report states that the facility did not ensure enteral feeding formula was labeled with the start time for one resident, did not administer the complete ordered dose of GT enteral feeding for two residents, and did not change one resident’s enteral feeding syringe daily. The report also states these failures had the potential for the residents to have complications related to the use of GT and/or to put the residents at risk for weight loss and/or dehydration. For one resident, the physician ordered Glucerna 1.2 at 45 mL/hr for 20 hours starting at 1200 hours, but during observation the GT feeding bottle was running without a start time label. The LVN stated the bottle should have been labeled with the resident’s name, ordered rate, and the date and time the feeding was started so staff would know when to discard it after 24 hours. The LVN verified the bottle was not labeled with the start time. For another resident, the physician ordered Glucerna 1.5 at 90 mL/hr for 12 hours starting at 1800 hours and later changed the order to 75 mL/hr for 15 hours starting at 1800 hours. The feeding bottle was observed without a start time label, and later the bottle was still labeled with the earlier rate and duration even though the order had changed. The feeding pump history showed the resident was still receiving the earlier ordered rate and volume, and the LVN verified the new rate and duration should have been changed. For a third resident, the physician ordered Glucerna 1.2 at 65 mL/hr for 20 hours to provide 1300 mL, but the feeding pump history showed the resident had not received the full prescribed amount, and the ADON verified the ordered dose had not been delivered. For the fourth resident, the physician ordered the enteral feeding syringe to be changed every day shift daily, but the syringe at bedside was labeled with a date several days earlier. The LVN verified the syringe had not been changed as ordered. The facility’s policies reviewed in the report stated enteral formulas should be administered as ordered, labeled with the required information including date and time prepared and hung, and enteral feeding syringes should be replaced every 24 hours.
