Failure to rotate injectable medication sites and complete readmission assessment
Summary
The facility failed to provide care according to physician orders for multiple residents whose insulin and other injectable medications were ordered to have injection sites rotated. For Resident 6, the record showed orders for regular insulin before meals and at bedtime and insulin glargine at bedtime, both with instructions to rotate the site of administration. The location of administration report showed repeated injections into the same LLQ abdominal site on multiple occasions. During interview, an LVN verified the orders and confirmed the sites were not rotated, stating the sites should be rotated to prevent complications from repeated injections at the same site. For Resident 7, the physician ordered insulin glargine at bedtime with instructions to rotate the site of administration. The location of administration report showed repeated administrations to the left arm on multiple occasions. During interview, an LVN reviewed the MAR and verified the insulin sites were not rotated, stating the sites should be rotated as ordered. For Resident 9, the physician ordered Humulin R insulin before meals and at bedtime with instructions to rotate the site of administration. The location of administration report showed repeated injections into the LLQ abdomen on multiple occasions. An LVN reviewed the MAR and verified the insulin sites were not rotated and stated the sites should be rotated. For Resident 230, the physician ordered Humulin regular insulin before meals and at bedtime, along with NPH insulin in the morning and at bedtime, with instructions to rotate injection sites. The location of administration report showed repeated administrations into the LUQ abdomen for both insulin types on multiple occasions. An LVN verified the findings and stated the injection site should be rotated with each insulin administration. For Resident 201, the physician ordered Lovenox in the morning and Lispro insulin before meals and at bedtime with instructions to rotate injection sites. The location of administration report showed repeated Lovenox injections in the LUQ and LLQ abdomen and repeated Lispro injections in the LLQ, RUQ, LUQ, and RLQ on consecutive administrations. The ADON verified and acknowledged that the injection sites for both the anticoagulant and insulin were not rotated. The facility also failed to complete an assessment for Resident 222 upon readmission. Resident 222 was readmitted from the hospital, was awake and alert but weak, and was to receive IV ceftriaxone for pneumonia. The record showed that later the same day the resident became unresponsive, had no detectable pulse or respirations, and was pronounced deceased. Review of the medical record failed to show documented evidence of the assessment and vital signs when the resident was readmitted three hours before the change in condition. The ADON stated the Admit/Readmit Evaluation should have been completed and initial vital signs should have been taken as soon as possible, and the DON stated a thorough assessment and initial vital signs should be completed for every admission or readmission.
