Summary

The facility failed to maintain an accurate facility assessment that evaluated the resources needed to meet residents’ care and service needs. The assessment dated 06/01/2026 included resident demographics stating the population had a socio-economic status of poor and sexual orientation of straight, even though the facility accepted residents of all socio-economic backgrounds and sexual orientations, as shown by private pay residents and a same sex couple living in the facility. The assessment also stated the facility would employ a RN or LPN with certification in Infection Prevention and Control on at least a part-time basis, but the facility staff list provided to surveyors showed no staff identified as the Infection Preventionist. The assessment further stated that the persons involved in completing or reviewing it included the DON and Medical Director, but interviews showed otherwise. The DON stated she had not seen the assessment and was not involved in reviewing it. The Administrator stated the Medical Director, DON, and Administrator were involved in reviewing the assessment, but later stated the assessment was inaccurate and needed to be redone from the start because of the inaccuracies in demographics and staffing. The Medical Director stated he did not know what a facility assessment was and was not involved in reviewing the current assessment. The Regional Nurse Consultant also stated the assessment was inaccurate and that both the DON and Medical Director should have been involved in reviewing it.
