PASRR NFSS Forms Not Submitted for Two Residents
Summary
The facility failed to coordinate assessments with the PASRR program to the maximum extent practicable for two residents by not incorporating PASRR level II recommendations and evaluation findings into assessment, care planning, and transitions of care. The deficiency involved Residents #1 and #2, both of whom were identified as PASRR positive and had care plans noting specialized services and wheelchair needs. Resident #1’s records reflected diagnoses including non-alzheimer’s disease and hemiplegia or hemiparesis, and Resident #2’s records reflected diagnoses including non-alzheimer’s dementia and seizure disorder or epilepsy. Resident #1’s quarterly MDS showed mild cognitive impairment and did not indicate wheelchair use, while her care plan stated she had an ID diagnosis and was PASRR positive, with specialized services and wheelchair replacement noted. Resident #2’s quarterly MDS showed severe cognitive impairment and also did not indicate wheelchair use, while her care plan stated she had ID and was PASRR positive, with specialized services and wheelchair repair noted. Their HSPs, both dated 04/22/26, reflected recommendations for services including DME and noted that a CMWC was requested, with outcomes listed as pending assessment. During interview, the DOR stated that a meeting in April 2026 determined both residents needed new wheelchairs and that the vendor initially came to measure them, but did not return to complete the process. The DOR said she had been communicating with the vendor and had not submitted the NFSS forms because she was waiting for the measurements to be completed, and she stated she did not know she only had 20 business days to complete and submit the forms. The DME vendor confirmed he had been unable to return to finish the measurements, and the residents’ RP and HC stated they were aware the facility was working on the wheelchairs but the forms had not yet been submitted.
