Failure to Timely Submit PASRR NFSS Request for Customized Wheelchair
Summary
The facility failed to incorporate the recommendations from the PASRR Level II determination and PASRR evaluation report into Resident #1’s assessment, care plan, and transitions of care. Resident #1 was a male with diagnoses including muscle wasting and atrophy, muscle weakness, unsteady gait, lack of coordination, reduced mobility, and unspecified intellectual disabilities. His quarterly MDS dated 07/02/2026 showed a BIMS score of 02, indicating severely impaired cognition, and also documented bilateral lower extremity impairment, wheelchair use, and need for partial/moderate assistance with lower body dressing, footwear, personal hygiene, toileting hygiene, and showering. Record review showed Resident #1’s care plan identified him as PASRR positive related to intellectual disabilities and included interventions for service coordination and discussion of any need to evaluate for habilitative services and/or durable medical equipment. The PASRR NFSS form reflected a request for a customized manual wheelchair, with the assessment dated 06/25/2026. The facility’s intake investigation worksheet also documented that the NF had failed to submit a complete and accurate NFSS request in the LTC Online Portal within 20 business days after the IDT meeting, and that the resident had not received a Medicaid service because the request was not submitted as instructed and was denied without a follow-up submittal. During interviews, the DOR stated that he and the DON were responsible for submitting NFSS requests through the LTC Online Portal and that the process began with an IDT meeting, therapy evaluation, and then submission of the NFSS request. He stated that Resident #1’s customized wheelchair request had not gone through because of a glitch in the system and that he later realized it had not been submitted. The DON confirmed that the IDT meeting was held on 04/21/2026, that a customized manual wheelchair was recommended, that the NFSS form was first opened on that date, and that the 20-business-day timeframe had passed. The DON also stated that the form was incomplete and that the DOR had difficulty submitting it.
