PASRR Screening and Referral Errors
Summary
The facility failed to coordinate assessments with the Pre-admission Screening and Resident Review (PASRR) program to the maximum extent practicable for four residents reviewed. Two residents had Form 1012s completed incorrectly after admission, and two residents developed new mental health diagnoses that were not referred for a new PASRR Level II assessment. The deficiency involved residents with diagnoses including bipolar disorder, depression, PTSD, schizoaffective disorder, psychosis, anxiety, and dementia. For one resident, the record showed a PASRR Level I screening was negative, but the Form 1012 was marked "yes" for mood disorder while also indicating that no new Level I screening was needed. The resident’s record reflected diagnoses of bipolar disorder, depression, generalized anxiety disorder, and PTSD, and the MDS showed intact cognition with active mental health diagnoses. A second resident had a similar issue: the PASRR Level I screening was negative, but the Form 1012 was marked "yes" for mood disorder and again indicated that no new Level I screening was needed. That resident’s record reflected diagnoses of major depressive disorder, bipolar disorder, obsessive-compulsive disorder, unspecified dementia, and other dementia-related psychotic and mood disturbances. Two other residents had later psychiatric diagnoses that were not referred for PASRR Level II assessment. One resident’s record reflected diagnoses of dementia, major depressive disorder, anxiety disorder, and unspecified psychosis, and a psychiatric subsequent assessment documented unspecified psychosis. Another resident’s record reflected dementia, hallucinations, and other schizoaffective disorders, with a psychiatric note documenting schizoaffective disorder, bipolar type. The MDS nurse stated the Form 1012s were incorrect, that new PASRR screenings should have been submitted for the residents with new diagnoses, and that she had not received training on PASRR services from the facility. The DON stated that if residents were not screened appropriately, they could potentially miss out on services.
