Incomplete Antipsychotic Consent Forms
Summary
The facility failed to ensure that residents were fully informed and that consent forms for antipsychotic medications were correctly completed for six residents reviewed. The deficiency involved HHSC Form 3713 for antipsychotic or neuroleptic medication treatment, where required signatures and dates were missing for the practitioner, resident, or resident representative on multiple forms. The report states that these incomplete forms were present while the residents were receiving antipsychotic medications such as Seroquel, Risperdal, Risperidone, Olanzapine, and Quetiapine. Resident #4 had diagnoses including bipolar disorder, depression, generalized anxiety disorder, PTSD, diabetes, acute kidney failure, and heart failure, and had a BIMS score of 15 indicating intact cognition. The resident had an active order for Seroquel 100 mg at bedtime for bipolar disorder. The consent form for antipsychotic treatment listed Seroquel as the proposed therapy, but the practitioner’s signature and date were missing, and the resident’s signature was not dated. Resident #56 had dementia, Alzheimer’s disease, major depressive disorder, anxiety disorder, and a BIMS score of 2 indicating severe cognitive impairment. The resident received Risperdal 0.5 mg daily for affective mood disorder, but the consent form had no signature or date from the person prescribing the medication or designee. Resident #73 had dementia with behavioral disturbance, bipolar disorder, psychosis, mood disorder, and schizoaffective disorder, with a BIMS score of 00 and dependence for multiple activities. Olanzapine was ordered and administered from April through June 2026, but the consent form was not signed or dated by the physician. Resident #82 had dementia, major depressive disorder, persistent mood disorder, and severe cognitive impairment, and received Risperdal 0.25 mg at bedtime; the consent form lacked dates next to the prescriber’s signature and the resident representative’s signature. Resident #104 had Alzheimer’s disease and anxiety disorder with a BIMS score of 03, and received Seroquel 25 mg daily; the consent form was not signed or dated by the physician. Resident #109 had bipolar disorder and schizophrenia with a BIMS score of 11 and received Risperidone 1 mg twice daily; the consent form was not signed or dated by the physician and was not dated by the resident.
