Summary

The facility failed to ensure that medications and stock supplements were properly dated when opened and discarded in a timely manner on two of five medication carts reviewed, including the Unit 100 and Unit 700 carts. Facility policy required medications to be administered as prescribed and expired medications to be discarded per policy, and the house-supplied medication policy stated that the manufacturer’s expiration date would be used to determine when a medication was to be disposed of. Manufacturer instructions for Insulin Lispro Kwikpen stated that the pen should be thrown away after 28 days, even if insulin remained in it, and instructions for UTI-Stat stated that it should be discarded three months after opening with the opening date recorded on the bottom of the container. On the Unit 100 medication cart, surveyors observed an opened Insulin Lispro Kwikpen without an open date, and the LPN confirmed that staff were unable to determine the discard date for safe use of the insulin. On the Unit 700 medication cart, surveyors observed an opened bottle of UTI-Stat with no open date, and the LPN confirmed that staff were unable to determine the discard date for safe use of the supplement. The DON later stated that it was the facility policy to always date insulin and supplements when opened to ensure safety when administering to a resident.
