Failure to Maintain Function and Provide Scheduled Bathing
Summary
The facility failed to provide necessary care and services to maintain a resident’s level of function and failed to complete routine bathing for that resident. Resident 6 stated that after therapy services ended, staff no longer used the sit-to-stand lift for transfers and instead used a full body lift, which led to the resident using a bed pan rather than being transferred to the toilet. The resident also stated that there were no exercises or programs provided after therapy to help prevent decline in transfer ability or continence. The record showed that the resident had previously been able to transfer with a sit-to-stand lift, propel a wheelchair independently for 150 feet, and had been only occasionally incontinent of bladder and continent of bowel, but later assessments documented partial to moderate assistance with personal hygiene and wheelchair propulsion and frequent bladder and bowel incontinence. Facility staff confirmed that there was no restorative nursing program in place. The PTA stated that when residents were discharged from therapy, the therapy department did not make recommendations for restorative nursing services because the facility did not have such a program. A nurse aide confirmed that Resident 6 had declined in function, including needing a full body lift and using a bed pan instead of the toilet. The DON also confirmed that the facility did not have a restorative nursing program or any program to support health maintenance activities to help prevent decline in function, and acknowledged that Resident 6 had declined in mood, personal hygiene ability, wheelchair self-propulsion, and continence. The facility also failed to provide bathing as preferred and documented. Resident 6 stated a preference for two baths per week but reported often receiving only one bath weekly and sometimes none at all. A progress note documented that the resident raised this concern during a care conference and that the care plan was changed, but the care plan reviewed later still reflected only one whirlpool bath weekly and did not reflect the resident’s stated preference for two baths weekly. Point of care documentation showed weekly baths on several dates, with no documentation of twice-weekly bathing and no bath documented after the last recorded bath. Staff reported that a scheduled bath was missed because of staffing issues, and the DON confirmed that the resident had not been bathed since the last recorded bath and had gone more than a week without bathing.
