Summary

The facility failed to conduct and document a facility-wide assessment that included the key components of staffing needs for each resident unit and for each shift. During interview, the DON reviewed the assessment and staffing numbers and presented Attachment C, which was a one-page document titled Staffing Numbers - CNA; staffing numbers for RN or LPN were not found. The ADON stated she calculated CNA needs but did not calculate nurse staffing needs, and the facility assessment did not include licensed nurse staffing needs for each resident unit or each shift such as day, evening, and night.
