Environmental Maintenance and Housekeeping Deficiencies

Ledgewood ManorWindham, Maine Survey Completed on 05-28-2026

Summary

The facility failed to adequately provide maintenance and housekeeping services necessary to keep the physical environment in good repair and in a clean, sanitary condition. During an environmental tour conducted by two surveyors on May 28, 2026, widespread use of heavy adhesive tape was observed as a permanent repair method and as a room-dividing barrier in multiple resident rooms and common areas, including Rooms 101, 102, 104, 105, 106, 109, 111, 112, 113, 114, 115, 116, 119, 121, and 129, as well as at the Wing 2 entry, the area between rooms, the floor outside another room, and around the Nurses' Station and restroom entryway. The tape was described as sticky, frayed, heavily soiled, and creating un-cleanable surfaces. Tape was also observed on handrails outside Rooms 103, 105, 109, and 113, on a bathroom and bedroom floor, and on a broken sink fixture in a resident room. The Dining Room rear door also had tape affixed to the floor. Surveyors further observed the corridor floor outside several rooms chipped down to bare exposed concrete, large areas of missing flooring in another hallway area, and chipped paint on the Dining Room rear door. Two water-stained, wet ceiling tiles were sagging outside the Laundry area. In the Whirlpool Room, the shower area had accumulated black grout and a heavily soiled floor surface. A sit-to-stand mechanical lift parked in the corridor outside a resident room was covered in visible dirt and debris. On May 29, 2026, the full list of environmental findings was compiled and emailed directly to the Administrator at his request.
