Summary

The facility failed to ensure appropriate foot care for a resident with diabetes mellitus and peripheral vascular disease. Resident #9 had severe cognitive impairment, was dependent on staff for personal hygiene, and required extensive assistance with personal hygiene per the care plan. Review of the medical record showed the resident’s toenails were overgrown, and a progress note documented on 06/30/2026 that the toenails were approximately 1/2 inch long and extended over the nail beds. The resident was not evaluated by the podiatrist during multiple facility podiatry visits, including visits on [DATE], 03/26/2026, 04/16/2026, and 06/25/2026. Observations on 06/30/2026 and 07/01/2026 showed bilateral 2nd and 3rd toenails approximately 1/2 inch long and curled over the nail beds, the left great toenail approximately 1/2 inch thick and black and brown, the right great toenail approximately 1/4 inch thick and yellow/brown, and the bilateral 4th and 5th toenails extending about 1/4 inch past the nail bed. Staff, including an LPN, the Clinical Coordinator/MDS LPN, the DON, and the resident’s hospice nurse, stated the resident needed podiatry evaluation and toenail trimming.
