Oxycodone Delivery Not Properly Accounted For

Summary

The facility failed to ensure a resident’s oxycodone was accounted for after delivery for one of three residents reviewed for misappropriation of property. Resident B had an active order for oxycodone 10 mg, two tablets via gastric tube every four hours as needed for pain, and the pharmacy delivered two packets of 30 tablets each, for a total of 60 tablets. The medication was signed for by LPN 1, and the packets and narcotic count sheets were taken to RN 1, who was caring for the resident that day. LPN 1 did not witness RN 1 place the medication into the locked narcotic box on the medication cart. When the night shift QMA reconciled the narcotic count, only one packet of oxycodone and one narcotic count sheet were present, even though the delivery documentation showed 60 tablets had been received. The QMA also observed that the number at the top of the narcotic count sheet had been altered from 60 to 30 with black ink. During later interviews, RN 1 initially admitted to using a black marker to change the number but then denied diverting any medication. The facility record also showed the controlled substance administration record and packing slip reflected delivery of 60 tablets, while the count sheet and label had been altered to show 30 tablets.
