Insufficient RN Coverage
Summary
The facility failed to ensure it used the services of a registered nurse for at least 8 consecutive hours a day, 7 days a week. The Facility Assessment dated 9/16/2025 documented a current census average of 31-36 residents, 3 wings with double occupancy, and a staffing plan calling for licensed nurse coverage of 1 RN and 1 LPN or 2 LPNs from 6 AM to 6 PM, and 1 RN or LPN from 6 PM to 6 AM. During interview on 7/7/2026, the Administrator stated the DON is usually in the building 8 hours a day but is sometimes off one day during the week, and the DON takes vacation days and has most holidays off. Review of schedules and payroll records from 6/1/2026 through 7/9/2026 showed multiple days without the required RN coverage. On 7/1/2026, the schedule showed no RN scheduled and the DON’s payroll reflected a clock-in from 9:18 AM to 4:30 PM, for 6.75 hours of RN coverage. On 7/6/2026, the schedule showed no RN scheduled and the DON was absent on holiday, leaving 0 RN hours. Additional examples included 6/1/2026, when the DON clocked in at 8:36 AM to 4:30 PM for 7.5 hours of RN coverage, 6/9/2026, when the DON was absent on vacation and no RN was scheduled, and 6/19/2026, when the DON clocked in at 8:11 AM to 4:30 PM for 7.75 hours of RN coverage. The DON stated she understood some of the facility assessment and that she comes in when she can if needed for coverage. The facility census sheet dated 7/7/26 documented 29 residents living in the facility.
