Summary

The facility failed to provide the services of a Registered Nurse for eight consecutive hours per day, seven days per week. On 7/4/26, the only RN on duty, V101, clocked in at 10:50 AM and clocked out at 12:59 PM for a total of 2 hours and 9 minutes. On 7/5/26, V101 clocked in at 10:00 AM and clocked out at 11:45 AM for a total of 1 hour and 45 minutes. The Human Resource Director confirmed that V101 was the only RN on duty on those dates, and the Regional Nurse Consultant stated that no agency RNs were used that weekend.
