Failure to Send Ordered UA and Urine Culture

Summary

The facility failed to provide laboratory services for one resident when a urinalysis (UA) and urine culture ordered by the MD were not sent to the laboratory for testing. The resident was admitted with diagnoses including urinary retention, benign prostatic hyperplasia, and UTI, and had intact cognitive skills for daily decision making as well as capacity to understand and make decisions. On 7/9/2026, nursing documentation noted blood-tinged urine, purulent output in the urinary tubing, and a tender abdomen, and the MD gave orders to change the Foley catheter and send a UA and urine culture. The urine was collected and stored in the specimen refrigerator, but the order was not entered into PCC and the tests were not sent. During interviews, an LVN stated the resident’s cloudy and red urine could indicate a UTI and that the MD responded by text message requesting a UA and urine culture. The Infection Prevention Nurse stated the UA and urine culture were not ordered or sent to the laboratory, and that the order should have been placed in PCC but was not. A concurrent review of the facility phone text messages confirmed the MD texted to change the Foley and send a UA and urine culture, while the Order Summary Report did not show those orders. Facility policy stated that physicians identify and order diagnostic and lab testing, staff process test requisitions and arrange for tests, and telephone orders must be documented immediately and implemented promptly.
