Kitchen sanitation, food handling, and hand hygiene failures
Summary
The facility failed to keep the kitchen, food preparation equipment, and food contact areas clean and sanitary during observations of the kitchen and meal service. During the initial kitchen tour, a swipe test of the ice machine where ice dispensed revealed brown and black specks on a white paper towel. The ice machine was documented as cleaned weekly, with the cleaning schedule showing it had been cleaned on 06/02/2026 and 06/09/2026. A black insect box was also observed on the window seal above the food preparation area and another beside the microwave on the food preparation surface, and an ant was killed on the counter during food preparation without sanitizing the area afterward. Food storage and equipment sanitation concerns were also observed. A pork roast in a shallow pan in the bottom of the left refrigerator was labeled 06/01/2026, and the Dietary Consultant stated that date was when it was pulled from the freezer. On follow-up observation, the pork loin was no longer in the refrigerator, and the Dietary Consultant stated it had been thrown out because it had been held too long. The Dietary Manager stated the pork loin had been in the refrigerator since June 1 and should have been cooked the prior day for lunch. In addition, a large pot used to fry foods was observed with a brown murky oily substance, brown debris floating on top, and heavy black buildup of charring on the inside and outside. On follow-up, the pot still showed amber and brown residue with heavy black areas and flaking buildup on the inside and outside, despite staff stating it had been cleaned. Hand hygiene and food handling practices were also observed to be inconsistent with sanitary standards. Dietary Aide #10 was observed filling cups, touching the outside of a plastic wrap box, touching cup rims with a bare hand, and covering cups with plastic without hand hygiene. During lunch service, the same aide touched clothing, glasses, coffee cup lids, the ice scoop, the ice machine, and items in the refrigerator without hand hygiene between tasks. The aide stated, "I don't wash my hands as often as I should. It does spread germs." The Dietary Manager stated staff were expected to wash hands when moving from dirty to clean tasks and when touching clothing, and the Administrator stated kitchen staff needed to follow the facility hand sanitizing policy. Facility in-services and policies reviewed addressed handwashing, sanitizing surfaces, avoiding contact with cup rims, and keeping kitchens free from insects, but the observed practices did not reflect those standards.
