Summary

The facility failed to ensure that the resident call light system remained functional and within reach for two residents who shared a room. The call light box in the room had detached completely from the wall and was dangling behind one resident’s bed, and the other resident’s call light button was found underneath his bed. The facility’s policy stated that the resident’s call system remains functional at all times. Resident #7 had diagnoses including bipolar disorder, schizoaffective disorder bipolar type, unspecified dementia without behavioral disturbance, acute and chronic respiratory failure, and repeated falls. His care plan identified communication problems, cognitive deficits, ADL self-care deficits, and high fall risk, with interventions to keep the call light in reach and encourage use of the bell for assistance. During observation, he stated his call light had been broken for 4-5 months and that he would holler if he needed help. He was not given an alternative means to call for assistance until a bell was later observed on his bedside table. Resident #28 had diagnoses including cerebral infarction due to thrombosis of the right posterior cerebral artery and unspecified dementia, with care plan interventions addressing communication problems, cognitive deficits, impaired ability to make self-understood, ADL self-care deficits, and fall risk. During observation, he was lying in bed and could not explain how he called staff if he needed help. CNA staff reported the call light system in the room was broken and that the issue had been noticed and reported, while the maintenance director and DON gave differing accounts about when they learned of the problem and whether it had been reported for repair. The facility maintenance request log for the prior six months did not show any request for service for the room’s call light system.
