PICC Dressing Left Loose During IV Antibiotic Administration

Summary

The facility failed to ensure professional standards of practice were followed in accordance with physician orders and facility policy for PICC line care for one resident receiving parenteral and intravenous therapy. The resident had diagnoses including wound infection and a stage 4 sacral pressure ulcer, and the care plan noted a chronic history of infection and a PICC line with dressing changes per orders/protocol. Physician orders directed that the PICC line dressing be changed weekly and as needed, and that the PICC site be monitored for signs and symptoms of infection. During observation, the resident's IV antibiotic was infusing through the PICC line while the transparent dressing edges were peeling away from the skin and curling upward, almost exposing the insertion site. RN A stated she saw the dressing coming off when she administered the IV antibiotic, but because the PICC line flushed without problem, she gave the medication and planned for the DON to change the dressing later. The DON stated she was responsible for changing the dressing and that the nurse had notified her it needed to be changed, but surveyors observed the condition before it was changed. Staff interviews reflected that nurses were expected to check the dressing every shift and during every antibiotic administration, and the facility policy stated the purpose of the procedure was to prevent catheter-related infections associated with contaminated loose or soiled catheter-site dressings.
