Dumpster Left Open and Surrounding Area Contained Debris
Summary
The facility failed to dispose of garbage and refuse properly for 1 of 3 dumpsters reviewed for food and nutrition services. On 01/27/2026 at 9:08 AM, surveyors observed Dumpster #2 left open by the overhead plastic lid. The surrounding dumpster corral contained approximately 15 wooden pallets, 1 plastic rolling trash bin, 1 fish tank, and various waste items on the ground, including food, gloves, plastic, an over-the-counter medicine bottle, briefs, bed liners, cans, and leaves. During interviews, Dietary A described the process for disposing of waste and stated staff were expected to close the dumpster after use and keep debris off the floor because it was cross contamination and could attract cockroaches, flies, mice, and cats. [NAME] A stated the dumpster had been dirty two days earlier and that it needed to be closed at all times to prevent odors and to keep animals and insects away. The Dietary Manager stated the condition of the dumpster on 01/27/2026 was not acceptable because it was left open and debris was on the ground around the dumpsters. Additional interviews with LVN C, Housekeeping, the Maintenance Supervisor, and the Administrator confirmed that staff from multiple departments used the dumpsters and were expected to close them after use. They also stated the area around the dumpsters needed to be free of debris. The facility policy titled Garbage Cans, Dumpsters and Grease Trap Use and Cleaning stated that outside trash dumpsters, doors and/or lids will be kept closed when not in use and surrounding areas will be free of litter and debris.
