Summary

The facility failed to provide an ongoing resident-centered activity program to meet the individualized activity needs of three sampled residents. Resident 1, who had a BIMS score of 14 and diagnoses including CVA, stated the facility had no engaging activity program, that she watched television or sometimes sat outside with other residents, and that there were no group activities in the evenings or on weekends. Resident 3, who had a BIMS score of 15 and preferences for listening to music, reading books, group activities, and attending religious services, stated the facility did not have a consistent activity program, that she watched television, and that she wanted outdoor activities such as garage sales and gardening that were not provided. Resident 1 also stated the activities were not frequent enough and were the same every day, leaving her bored because they were not engaging. Resident 2 had no speech, rarely or never understood, and had diagnoses including Alzheimer's dementia. Her activity preferences included listening to music, spending time outdoors, and participating in activities, but she was observed lying awake in bed and was described by CNA 1 as nonverbal, always in bed, and not attending activities. The facility's activity calendar showed that activities ended at 3:30 p.m., and RN/Admin stated residents could watch television in their rooms, but could not provide a care plan addressing Resident 2's activity needs. RN/Admin also stated activity staff was not available, and CNA 2 stated he was assigned to provide residents exercises and trivia activities but had no training to provide activities.
