Failure to Report, Assess, and Monitor After Fall and Abuse Allegation
Summary
The facility failed to follow professional standards of nursing practice for two residents after separate changes in condition. One resident, admitted with diagnoses including type 2 diabetes, COPD, spondylopathy, and atrial fibrillation treated with apixaban, was identified as a fall risk and had care plan interventions to monitor for changes in condition and delayed injury for 72 hours. After an unwitnessed fall in the shower room, the resident reported that a CNA assisted him into the shower, stepped out, and that he slipped from the shower chair and struck his back and buttocks. The CNA stated she saw the resident on the floor, pulled him up by the arm using a gait belt, returned him to the shower chair, dressed him, and transferred him to the wheelchair without calling for assistance or notifying licensed nurses. The record and staff interviews showed the fall was not reported to licensed nurses at the time, and the resident was moved before a nurse assessed him. The RN and DON stated there was no documented evidence that the resident was assessed and monitored on multiple shifts after the fall, including neuro checks required for an unwitnessed fall. The DON stated the resident’s fall should have been reported to licensed nurses and that moving him before assessment could cause more injury. The DON also stated the resident was on an anticoagulant medication that could cause undetected internal bleeding following an unwitnessed fall. A second resident, admitted with type 2 diabetes, age-related osteoporosis, and muscle weakness, had intact decision-making and required extensive assistance with most activities of daily living. Staff interviews showed that after the resident alleged physical abuse by a CNA, including being punched in the face and rough handled during care, the licensed nurses did not document assessment and monitoring in the medical record. The nurses also did not notify the attending physician or family about the allegation. The RN stated there was no documented evidence of assessment and monitoring related to the allegation of abuse, and the DON stated the resident was not assessed and monitored following the allegation and that the concern was not handled as a change in condition at the time it was reported.
