Summary

The facility failed to maintain the functionality of one of two water heaters used to supply hot water to resident rooms, showers, the laundry, and the kitchen. A resident reported that the water was not always hot and stated the water heater occasionally tripped out and had to be reset by the receptionist's husband for hot water to return. The resident was unsure how long this had been occurring but said it had been going on for a while. During the survey, hot water was observed in the resident's room when the water was run. Staff interviews confirmed the water heater repeatedly went into lockout and required manual resetting. The Maintenance Director stated the unit had been cleaned in early 2024 and that the frequency of lockouts had decreased, but it still needed to be reset about once a week. Other staff stated the only way to know the water heater had tripped was to run the water and find it cold, and that several people knew how to reset it. The EVS manager stated the water heater tripped approximately every four to six hours and that on weekends the facility sometimes did not have hot water because not all CNAs knew how to reset it. The Administrator stated the water heater took about ten minutes to restore hot water after being reset, and current inspection certificates were available.
