Unclean resident room and improperly covered shattered window
Summary
The facility failed to ensure Resident room [ROOM NUMBER] was kept clean and sanitized. On observation, the room had scraps of torn paper along the floorboards of two walls, spilled milk, pieces of scrambled eggs, and a dirty napkin on the bedside table, and additional food debris and a dirty napkin on the floor under the table. The bed had crumbs of food and unknown stains. The resident assigned to the room was not present during the observation and was described as non-interviewable. An LVN stated the mess appeared to be from breakfast and said housekeeping had not yet made its round to the secured unit, though she acknowledged she could have called housekeeping sooner and that the resident deserved a clean room and dignity. The room was later observed to be clean during several checks, but it was again found unclean later the same morning with the same debris, spilled food, and stained bed linens still present. The Housekeeping Supervisor stated the housekeeper assigned to the secured unit started on another hall first and then ended in the secured unit, and that if nursing called about a room needing cleaning, it would be prioritized right away. He also stated that beginning the next day, the housekeeper assigned to the secured unit would start there first. The Administrator stated housekeeping usually cleaned the room in the morning and afternoon, and that the resident sometimes became aggressive and would not allow housekeeping to clean, though distraction was used and housekeeping did clean the room. The facility also failed to properly cover a shattered window in the same resident room. The window had shattered over the weekend, and staff cleaned up the glass and covered the opening with drywall. The Maintenance Assistant stated he did not leave a 4-inch opening for ventilation because he was concerned glass could fall and hurt someone, and he told staff to leave the door open for ventilation. The Maintenance Director stated only drywall was available at the time and that he was not aware the required opening had not been left. He also stated he did not know when the window would be repaired and had not obtained an estimate or invoice. The Administrator stated the resident was not moved because a single occupancy room was required and none were available, and later stated the window had already been replaced.
