Summary

The facility failed to establish and implement an infection prevention and control program for two residents observed for infection control issues. One resident had diagnoses including COPD, chronic respiratory failure, functional quadriplegia, and type 2 diabetes mellitus, and was dependent on staff for toileting, showers, and personal hygiene. During observation, an LVN administered an injectable diabetes medication without washing her hands or using hand sanitizer before putting on gloves and did not perform hand hygiene after the medication administration. The LVN later stated she did not perform hand hygiene before or after giving the medication and acknowledged that she should have done so to prevent contamination and infection. The DON stated that handwashing or hand sanitizer should occur before and after medication administration and that failure to do so could cause infection or cross-contamination of the resident or staff. The facility’s hand hygiene policy stated hand hygiene is the primary means to prevent the spread of infection and required hand hygiene before preparing or handling medications and after removing gloves. Staff interviews also reflected that hand hygiene was expected before entering and after exiting a resident’s room and before and after providing care. A second resident had diagnoses including acquired absence of other genital organs, type 2 diabetes mellitus without complications, and dependence on renal dialysis. The resident had care plan concerns for skin integrity related to a surgical wound and a heel wound, and active orders for topical antibiotics to the penile wound and left heel wound. Observation of the resident’s room showed no signage indicating enhanced barrier precautions. An LVN stated the resident was on EBP due to wounds and that signs were supposed to be posted on the door, while the DON stated the resident was supposed to have EBP signage outside the room and that the facility needed to do a better job ensuring signage was in place for anyone on EBP.
