A sign was posted on the activity room door stating that a meeting was in progress. During an interview on 06/02/26 at 10:50 a.m., the 8 residents stated the resident council met in the activity room and that staff came in and out without privacy at times, causing them to filter what they said because staff were present. They stated they could not recall how often the meetings were interrupted, but knew it occurred. The Activities Director said she had only been employed for a month, had been told the meetings were held in the activity room, and had posted a sign on the door for the one meeting held since her employment. She said she was not aware of staff coming in and out, and stated staff or visitors could only attend if invited. LVN E stated the meetings were closed and staff should not come unless invited, but she and the doctor entered because he needed to see 2 residents and one resident had an allergic reaction. The Administrator stated she was not aware of staff coming into the resident council meetings and confirmed staff had to be invited or it was against resident rights.

During an interview, Resident #18 stated that a nurse aide would lay on her in bed at night, describing that the aide laid across her stomach with her feet still on the ground on her right side to rest. A nurse aide later stated the resident had told her that another nurse aide had laid on her in bed and that the resident did not like it. The nurse aide said she had been trained to report concerns and grievances to administration, but she did not report this concern or file a grievance because she did not believe the allegation was plausible. The DON and Administrator both stated that a staff member receiving a resident report that another staff member had lain on the resident was required to notify administration.

During the Resident Council interview, residents stated their concerns were not addressed and that they were told the issues were being reviewed, but nothing was ever done, making them feel like they did not make any difference.
