Summary

The facility failed to identify Resident #32’s risk for skin breakdown and failed to implement effective preventive measures before a pressure injury developed on the mid back. Resident #32 had a history of dementia, displaced fracture of the left femur, moderate protein calorie malnutrition, adult failure to thrive, stage 2 chronic kidney disease, vision impairment, incontinence, impaired cognition, pain, fragile skin, and severe cognitive impairment. The plan of care and Braden Scale identified the resident as at risk for altered skin integrity, and the resident also had kyphosis and poor nutritional intake noted by the PA. On 03/02/26, the DON was called to the resident’s room after CNA #110 noticed the mechanical lift pad handles were bunched up behind the resident’s back while she was being transferred back to bed. The DON documented that the area on the back appeared dark like a bruise with a yellowish area where the lift pad straps appeared to rub. The facility identified the injury as caused by the mechanical lift pad straps, and the wound assessment later described a new unstageable pressure injury on the mid back measuring 8.4 cm by 7.8 cm with 70 percent slough and 30 percent eschar. The wound physician later documented the injury as a pressure injury caused by a medical device, and classified it as a stage IV pressure injury with undermining and exposed tendon. Staff interviews indicated the resident often sat in a wheelchair, moved and fidgeted frequently, leaned forward and side to side, and had an abnormal curvature of the back, but there was no protection in the area to prevent skin injury while she was in the chair. The IDT note included a pillow between the resident and the lift pad when up in the wheelchair, but the report states the facility failed to timely identify the pressure injury until it reached an advanced stage.
