Inaccurate wound care documentation and signed-off treatments

Summary

The facility failed to ensure medical records contained accurate documentation for one resident with complex medical needs, including diabetes, chronic renal failure requiring dialysis, prior amputations, gangrene, and a recent hand amputation. The resident was cognitively intact, dependent on staff for all ADLs, and had a surgical wound. Physician orders and the after-visit summary required frequent wound care and wound assessments for the right hand and dressing changes for the left upper extremity, with monitoring for infection and other wound changes. Review of the TAR and treatment notes showed multiple instances where staff signed off wound care and wound assessments as completed even though the care had not been done. On 06/15/26 at 6:00 A.M., an LPN documented that treatment was not completed and the following shift was informed, but there was no reason given and no further documentation that the treatment was completed that day. Similar documentation appeared for the right-hand wound assessment and the left upper extremity dressing change, with staff later signing the tasks as completed on subsequent shifts. The resident stated the right-hand dressing had last been changed on 06/14/26 at 3:00 P.M., and he reported that nurses frequently did not complete the ordered wound care. The DON and ADON/LPN verified that the documentation was inaccurate. They confirmed the right-hand dressing had not been changed since 06/14/26 at 3:00 P.M., despite entries showing it had been completed on later shifts. They also verified the left upper extremity wound had not been assessed because the dressing had not been removed since 06/14/26 at 3:00 P.M., even though staff documented the assessment as completed. In addition, a skin evaluation dated 06/16/26 was found inaccurate because it recorded measurements of the right wrist while the dressing was still in place, and the facility policy required documentation to be objective, complete, and accurate.
