Summary

Medications were not administered according to physician orders for three residents reviewed. One resident with a right humerus fracture, type 2 diabetes, and hyperlipidemia had Lipitor 10 mg given twice in one day instead of at bedtime, and ordered magnesium citrate for constipation was not given on the ordered date; the physician was not notified of the error or delay. The DON verified the magnesium citrate was not administered as ordered and that the Lipitor dose was given at two separate times on the same day after the order had been altered to reflect a time change. A second resident with hemiplegia, stroke, and type 2 diabetes had orders for insulin glargine nightly and Humalog sliding scale before meals and at bedtime, but the Humalog order and blood sugar check schedule were not transcribed on admission. The MAR showed Humalog sliding scale was not signed off as administered on two scheduled checks, and the resident’s blood glucose was later documented at 500 mg/dL. The DON stated the omission was discovered during a medication check, the physician was notified, and the resident had multiple insulin changes during the hospital stay with an A1C of 10.8 percent. A third resident with cervical spine fusion and intact cognition had orders for gabapentin and oxycodone for pain. The resident reported the facility had not received the pain medication on arrival and had to wait for it to arrive. Observation of the narcotic count sheet showed the oxycodone was delivered later, and the DON verified the nurse did not use the onsite emergency supply but borrowed the medication from another resident because the correct strength was not available in the cube. The DON stated borrowing from another resident was not allowed, and the facility policy required medications to be administered in accordance with the orders, including the time frame.
