Summary

Provide appropriate foot care was not met for one resident who was reviewed for foot care. The resident was admitted with diagnoses that included end stage renal disease and had a quarterly MDS assessment showing moderate cognitive impairment, dependence on staff for personal hygiene, and no moods or behaviors. On 4/22/26, the physician documented that the resident reported overgrown and protruding toenails causing discomfort and difficulty with trimming, and the physician ordered a podiatry consult for nail care and evaluation. The resident’s orders reflected the podiatry consult, and Nurse #2 was documented as the nurse who confirmed the order. Despite the referral, the resident was not seen by podiatry when the facility’s in-house podiatrist visited on 5/28/26. The Administrator provided a list of residents seen by podiatry that day, and the resident’s name was not on it. During interview, the resident stated he had told the physician he needed podiatry and had heard nothing since the referral was discussed. On observation, the resident’s left third toenail extended approximately 3/4 of an inch away from the toe and the toenails had not been trimmed. Nurse #2 stated she did not remember confirming the order and did not inform the SDC that the resident needed podiatry. The SDC stated she was not aware of the referral, and the Administrator and physician both stated the resident should have been seen by podiatry either during the in-house visit or through an outside appointment.
