Failure to Honor Resident Shower Preferences
Summary
The facility failed to promote and facilitate resident self-determination when staff did not honor reasonable shower preferences for three cognitively intact residents who required assistance with bathing. The facility policy titled Resident Bathing stated staff shall provide person-centered care emphasizing residents’ comfort, independence, and personal needs and preferences. Facility staff, including CNAs, LPNs, the DON, and the Administrator, stated residents should receive showers two times per week, and that refusals should be documented and residents reapproached if they refused. Resident #43 had diagnoses including Parkinson’s disease and colostomy status and required extensive to total assistance with transfers and bathing. The resident’s shower records showed showers on 03/05/26 and 03/26/26, then on 04/03/26, 04/09/26, 04/16/26, and 04/24/26, with a refusal documented on 05/15/26 and no additional showers documented for May. During interview, the resident stated showers were occurring about every two weeks, that the shower aide said a shower would be provided when help with the Hoyer lift was available, and that the resident preferred showers twice per week because he/she was a big person and felt dirty from sweating. The resident later stated he/she had not received a shower that week and did not know whether there was a set schedule. Resident #25 had diagnoses including Parkinson’s disease, anxiety disorder, traumatic brain injury, osteoarthritis, and below-the-knee amputation, and was cognitively intact and dependent on staff for showers. Shower records showed showers on 03/02/26 and 03/21/26, then on 04/01/26, 04/09/26, 04/17/26, and 04/23/26, and on 05/01/26 and 05/15/26, with no refusals documented. During interview, the resident stated he/she only got one shower every two weeks and wanted at least one shower per week, did not want to smell, and had not had a shower recently. Observation showed oily hair and visible flakey particles. CNA D stated he/she was not aware of the resident’s shower preference, and CNA/Shower Aide F stated the resident refused showers at times but no refusals were documented from 03/02/26 to 05/15/26. Resident #3 had diagnoses including cellulitis of the right lower limb, weakness, and anxiety, and was cognitively intact with substantial to maximal assistance needed for toileting hygiene and showering. The care plan noted the resident was resistive to care at times and staff should encourage participation, but it did not address shower preference or refusals. Shower records showed showers on 03/06/26, 03/11/26, and 03/24/26; on 04/03/26, 04/09/26, 04/15/26, 04/22/26, and 04/30/26; and on 05/06/26, 05/11/26, and 05/19/26, with no refusals documented. The resident stated he/she preferred two showers per week to feel clean and did not like feeling dirty or worrying about body odor. Staff interviews confirmed the facility expected two showers per week, that refusals should be documented in the shower sheet and electronic plan of care, and that residents should be reapproached if they refused.
