NA I was assigned to resident halls A and E and was observed providing incontinence care for Resident #7 and transferring the resident with a mechanical lift; during interview, NA I said he/she had worked as an NA for eight months, had just completed CNA class, and was waiting to take the certification test. NA K was scheduled and assigned to work as an NA after returning from extended leave, and the Administrator stated NA K had not been added back to the class. The Administrator also stated NA J was supposed to take the test but the test shut off, and NA I had completed classes but had not scheduled the test yet.

The interim ADMN stated the aide should have been termed if unable to get certified within 4 months and stated the aide was termed on 06/24/2026 when the issue was discovered.

The Administrator said NA A was terminated for about one week and then rehired on a part-time basis while completing the online class, and the DON and ADON said the ADON monitored completion of online modules and knowledge tests while the aides worked toward certification.
