Summary

The facility failed to ensure RN coverage for at least 8 consecutive hours a day on 2 of 30 days reviewed for RN coverage. A review of the Daily Staffing Pattern sheets dated 4/11/26 to 5/12/26 showed that RN/LPN hours were not separated by licensure, and the line labeled Total Hours (RN) reflected total nursing hours worked rather than RN hours only. The facility’s employee list showed that both RNs and LPNs were employed, but the staffing sheets did not distinguish between them. During interviews, the NHA stated she believed the Total Hours (RN) line represented only RN hours and was not aware that all nursing hours were included there. After the facility provided a breakdown of RN and LPN hours, it was determined that there was no RN coverage on 4/26/26 and 5/9/26. UM C later confirmed there were no RNs on those two days, stating that on 4/26/26 the RN had PTO and they did not catch it, and on 5/9/26 an RN was scheduled but did not report to work after mixing up the schedule.
