Summary

The facility failed to protect residents from the wrongful use of their belongings or money by not safeguarding controlled narcotic medications. During review of a self-reported incident involving a resident with an order for oxycodone for severe pain, surveyors found that 3 pills were missing from the narcotic count. The resident had a BIMS score of 15 and stated that the last pain medication was given by a previous nurse, while the nurse assigned to the cart could not account for the missing pill. The MAR did not document oxycodone administration by that nurse, and the discrepancy was identified during a narcotic count between staff members. A second incident involved another resident whose oxycodone bubble packs were found missing during a shift handoff and narcotic count. Staff reported that the number of narcotic cards on hand did not match the expected count, and the nursing supervisor found a narcotic sheet for oxycodone with tablets lined through without an explanation or signature for destruction. The resident had a BIMS score of 15 and had orders for oxycodone tablets for scheduled and as-needed pain management. Review of the MAR showed no documentation of PRN oxycodone administration, while the narcotic count sheet reflected multiple administrations and later showed tablets marked as destroyed with witness initials. Further review of the facility’s investigation found that narcotic cards were deducted from shift count sheets on dates worked by one nurse, and some narcotic administration count sheet signatures and initials did not match any staff members. The NHA, ADON, and Unit Manager stated they believed drug diversion occurred based on their investigation of both incidents. The surveyor determined that the incidents were a concern for misappropriation of property because the medications had been prescribed and delivered to the residents but could not be accounted for.
