Summary

The facility failed to accurately identify each resident’s fall risk status by not completing the Fall Risk Assessment for 3 of 3 sampled residents. The facility’s policy stated that the risk assessment was to be completed by the nurse or designee upon admission, quarterly, or when a significant change was identified, and that it should identify environmental hazards and individual risks, including the need for supervision. Review of the records showed that Resident #1 had an admission on 05/18/2026 and a readmission on 06/03/2026 with diagnoses including age-related physical debility, iron deficiency anemia, gout, falls, pain, multiple fractures, cachexia, and lung nodules. After an unwitnessed fall with injury on 05/27/2026, the readmission Fall Risk Assessment was incomplete, with missing sections for balance, sitting, transfers, systolic blood pressure, age, health conditions, and medications, and no fall risk score or level of risk identified. Resident #2 was admitted on 05/27/2026 with diagnoses including Type 2 diabetes, essential hypertension, and anxiety disorder. The admission Fall Risk Assessment was incomplete because the mobility section was not completed, and no fall risk score or level of risk was identified; the resident later had an unwitnessed fall without injury on 05/31/2026. Resident #3 was admitted on 06/02/2026 with diagnoses including Type 2 diabetes and a non-displaced fracture of the fourth metatarsal, and the admission Fall Risk Assessment was incomplete because the continence section was not completed, with no fall risk score or level of risk identified. During interviews, the MDS nurse acknowledged the incomplete assessments for Residents #2 and #3, the DON acknowledged that all three residents’ Fall Risk Assessments had not been completed and the fall risk statuses were not identified, and the Administrator also acknowledged the assessments had not been completed.
