Misappropriation of Resident Money and Blank Checks

Pleasant Meadows Senior LivingChrisman, Illinois Survey Completed on 06-26-2026

Summary

The facility failed to ensure a resident’s right to be free from misappropriation of money and blank checks. R4’s MDS documented a BIMS score of 13 out of 15, indicating no cognitive impairment. R4’s care plan identified her as at risk for psychosocial well-being problems, including low risk of abuse related to dependence on others, anxiety, suicidal ideations without a plan, sad mood, self-isolation, and sensory impairments including glaucoma, glasses, and hearing aids. The grievance log documented that R4’s friend and independent caregiver filed a complaint on her behalf regarding the alleged misappropriation of $100 and four blank checks. The facility’s investigation, completed by the previous interim administrator and the director of clinical services, documented that the money and checks were reported missing sometime between March 1 and April 1, 2026. R4’s family member stated she had visited from Florida and placed the money and four blank checks into R4’s wallet on either March 27 or March 28, 2026. A local police report documented the allegation was reported by the facility on April 1, 2026, and stated the facility had not provided requested information, including a list of individuals with access to R4’s room and belongings or a summary of the internal investigation. Interviews showed that R4’s friend found her purse in the bottom drawer of her dresser with every compartment unzipped, including her makeup bag, and no money or checks inside. The friend reported this to R4’s daughter and the DON. The police chief stated the investigation could not go further because the facility had not provided the names and contact information of staff who cared for R4 after the daughter returned to Florida and before the missing items were discovered. R4 stated she was bothered that she could not figure out who stole from her, and a longtime friend said R4 was sharp, honest, and reliable and had told her someone had taken money from her purse while she was sleeping.

Short Summary

A CNA/QMA accepted a resident’s offer to cash scratch tickets in exchange for part of the winnings and also took a discharged resident’s Mounjaro injection for personal use. The resident with intact cognition confirmed the money arrangement, and an RN administered the unlabeled injection after the CNA/QMA asked her to do so. The facility’s policies prohibited staff from accepting resident money or gifts and defined diversion of a resident’s medication for personal use as financial abuse.
