Summary

Facility failed to provide a resident with proper equipment, specifically a wheelchair, in accordance with therapy recommendations and the resident’s assessed needs. The resident was admitted with diagnoses including stage 5 chronic kidney disease, diabetes mellitus, and bipolar disorder, and had intact cognition with a BIMS score of 14. The admission functional assessment identified the resident as dependent on staff for transfers, mobility, and wheelchair needs, and the care plan included assistance with all ADLs, including transferring, mobility, and ambulation. During observation and interviews, the resident stated he had been without a wheelchair since admission and had been using a specialized chair for all needs. The specialized chair was observed at the bedside. The Director of Rehab/OT stated the resident was not currently in therapy, did not ambulate, and had been discharged from therapy after reaching full potential. The Director of Restorative stated the resident was non-ambulatory and used a specialized wheelchair when out of bed, for dialysis, and locomotion, but also stated the resident had not been assessed for wheelchairs and had never had a manual wheelchair. Further interviews showed conflicting understanding among staff about the resident’s equipment needs. The Director of Rehab later stated he had requested a lightweight wheelchair and that the specialized chair did not help the resident because he did not perform anything while in it. The DON stated the facility orders and provides DME when residents are assessed to need it, and acknowledged that special chairs cannot be used as a means of locomotion while wheelchairs can be used. Therapy records showed the resident had participated in PT and OT, reached maximum functional potential, and had discharge recommendations for functional maintenance and restorative nursing, with community mobility documents referencing wheelchair use and management.
