Summary

The facility failed to ensure that residents who chose to stay in their rooms were offered activities for 4 of 4 residents reviewed for activities. R64, admitted 1/11/24 with diagnoses including chronic respiratory failure, major depressive disorder, heart failure, anxiety, and morbid obesity, stated that she had not received any activities she could remember and that she preferred to remain in her room rather than be transferred to a chair for dining room activities and meals. She reported that she used to receive puzzles, coloring items, and reading materials, but could not remember the last time they were provided, and said she would like to receive them again. R66, admitted 2/6/19 with unspecified dementia, COPD, and alcohol abuse, stated that he did not get daily delights or puzzles passed out to him and said he would like to receive word searches and puzzles to work on in his room or in the hall. R22, admitted 6/16/25 with anxiety, CHF, diabetic neuropathy, obesity, and anxiety disorder, stated that he did not get any daily handouts with activities and wanted to see what they were. R80, admitted 12/21/23 with generalized anxiety disorder, chronic respiratory failure, hypertension, morbid obesity, and urinary retention, stated that he did not recall ever receiving daily handouts with reading materials and puzzles and would like to see them and possibly work on some puzzles. The Activities staff member stated that daily delights, puzzles, and coloring pages were passed out in the dining room and also to residents who stayed in their rooms, but that there were days she did not make it to all rooms because she got behind. The Administrator stated that all residents should be receiving the activity packets whether they attended planned activities or not.
