Failure to Complete Post-Fall Assessment and Monitoring
Summary
The facility failed to timely identify a right hip fracture for a resident who had an unwitnessed fall and later reported hip pain. The resident had severe cognitive impairment with a BIMS score of 5, along with multiple serious wounds, severe protein calorie malnutrition, palliative care status, need for assistance with personal care, and muscle weakness. After the fall, the resident’s communication log with hospice documented hip pain, but the post-fall evaluation did not include a comprehensive assessment, did not document factors that may have contributed to the fall, and did not reflect a detailed musculoskeletal assessment, assessment for signs of injury, review of fall risk factors, review of medications associated with falls, or documentation of how the resident was found on the floor. The resident’s SBAR communication form also did not reflect a complete physical assessment, with behavioral, respiratory, cardiovascular, abdominal, genitourinary, skin, pain, and neurological sections marked not clinically applicable. Nursing staff did not complete the required neuro checks within the facility’s protocol for unwitnessed falls, including multiple checks on the day of the fall and the required shift checks on the following days. The neuro check documentation that was completed was inconsistent, including one entry that marked no signs of pain while also documenting a pain score of 10. Progress notes showed no interdisciplinary documentation after the fall to identify possible causes or address the resident’s pain and comfort needs. The resident later screamed in pain, and hospice increased pain medication orders after being contacted by the facility. The physician later observed a deformity of the resident’s right hip, prompting x-ray imaging that confirmed an acute intertrochanteric hip fracture with moderate displacement, identified seven days after the fall. The resident’s weekly nursing summary due during this period was not completed, and the DON acknowledged that the missing weekly summary and neuro checks resulted in missed critical information regarding changes in condition.
