Summary
The facility failed to use the services of an RN for at least 8 consecutive hours a day, 7 days a week. Review of PBJ FY Quarter 2, 2025 showed no RN hours on every day from January 1, 2025 through February 28, 2025. Review of PBJ FY Quarter 3, 2025 showed no RN hours on April 6, 2025, May 31, 2025, June 7, 2025, June 8, 2025, and June 14, 2025. Facility timeclock punch details reviewed for May 31, 2025 through June 14, 2025 showed no RN on duty for 8 consecutive hours on May 31, June 1, June 7, June 8, and June 14, 2025. Timeclock punch details reviewed for March 1, 2025 through March 6, 2025 showed no RN on duty for 8 consecutive hours on March 1 and March 2, 2025. During interviews, the Ombudsman reported resident concerns about long call light wait times and call lights not being answered in the evenings. An RN stated she was responsible for an average of 15 to 16 residents and had cared for as many as 23 residents, and said she had never been the sole nurse working at the facility. The DON stated staffing was based on acuity, census, and state standard PPD, and that the facility had an alternate staffing plan and scheduled an extra nurse for weekend coverage. He also stated there were no days when there were no licensed nurses available to provide care, but acknowledged that the facility did not have access to all requested punch details and that the staffing concerns inherited from the previous owner did not meet expectations. Facility policy required sufficient nursing staff with appropriate competencies and skillsets based on resident assessment, care plans, number, acuity, diagnoses, and facility assessment.
