Summary

The facility failed to ensure a resident was protected from the wrongful use of his belongings or money when 38 hydrocodone-acetaminophen tablets were found unaccounted for. The resident was a male admitted with diagnoses including an unspecified subluxation of the right patella and an anxiety disorder. His assessment reflected short-term memory recall, some difficulty making decisions in new situations, and use of PRN pain medication. His care plan identified pain medication therapy related to his knee injury, with interventions to administer medication as ordered and monitor for adverse reactions and falls. The resident had physician orders for hydrocodone-acetaminophen, including a supply delivered to the facility and later revised orders. The provider investigation report stated that on 05/08/2026 it was discovered that 38 hydrocodone tablets were unaccounted for, along with the narcotic count sheet. The facility reported that it initiated its drug diversion protocol, interviewed staff and residents, performed drug testing, audited narcotic shift change sheets, notified the medical director, pharmacy consultant, physician, and police, and began a card count sheet. The facility also stated that it could not locate the missing narcotics and could not identify an eyewitness to the medications being taken. Record review and interviews showed that nursing staff had counted narcotics by reviewing the count form but did not actually look at the medication blister card. The DON stated the last known time the hydrocodone was seen in the medication cart was several days before the discovery, and that the medication was missing when the resident returned from the hospital and the cart was checked. Staff interviews reflected that the narcotic sheet was also missing, and the facility could not determine who had access beyond staff assigned to the carts on their shifts. The resident stated he had no concerns about not receiving his pain medication and reported that staff gave him pain medication when requested and that it helped his pain. His pain assessments during the review period showed pain levels ranging from 6 to 8 on a 10-point scale.
