Summary

Three of three exhaust fans in the shower rooms were not operational when the facility was observed with the Maintenance Supervisor on May 7, 2026. The shower rooms were inspected during the survey, and the Supervisor acknowledged that the exhaust fans were not working. A faucet in Resident 1’s room was continuously leaking and creating a constant noise disturbance. Resident 1’s clinical record showed diagnoses including acute respiratory failure with hypoxia, asthma, and aphonia. During interview, Resident 1 stated the faucet had been leaking for about two months and had disrupted her sleep at night, and she reported that staff had told her it was on the maintenance list. During observation with the Supervisor, the faucet near Resident 1’s bed was still leaking. The ADON reviewed the facility’s Maintenance Service policy, which stated maintenance service shall be provided to all areas of the building, grounds, and equipment, and acknowledged that the policy was not followed.
