Summary

The facility failed to ensure trauma-informed care was implemented for a resident with a documented diagnosis of PTSD. Resident #6 was admitted with multiple psychiatric and medical diagnoses, including cerebral palsy, schizophrenia, schizoaffective disorder bipolar type, generalized anxiety disorder, major depressive disorder, intellectual disability, and PTSD. A psychiatric evaluation documented that the resident reported a history of sexual trauma beginning in childhood, experienced flashbacks and nightmares every other day and night, and identified interactions with male individuals as a trigger that caused increased anxiety, fear, and emotional distress. Review of the resident’s MDS showed the resident required staff assistance with activities of daily living, including transfers, mobility, toileting, bathing, and personal hygiene. The comprehensive care plan addressed anxiety, depression, psychosis, and schizoaffective disorder, but did not include interventions for PTSD triggers or the resident’s preference for female caregivers. During interviews, the resident stated she was unsure whether the facility was aware of her triggers and later reported that male interactions could trigger PTSD symptoms and that she preferred female caregivers. CNA #134 stated they were not aware of the PTSD diagnosis or any triggers related to care, and the DON was unable to locate documentation identifying PTSD triggers, including interactions with male staff providing care.
