Failure to Use Enhanced Barrier Precautions During Resident Care

Summary

The facility failed to use enhanced barrier precautions for Resident #54 during high contact resident care. Resident #54 was admitted on 12/31/20 and had diagnoses including hemiplegia and hemiparesis following cerebral infarction affecting the right dominant side, dysphagia, chronic obstructive pulmonary disease, and pulmonary edema. Her care plan and physician orders identified her as requiring enhanced barrier precautions because she had a feeding tube, and the plan directed staff to use gloves, gown, mask, and face shield during high contact care such as dressing, bathing, hygiene care, toileting assistance, and transfers. The quarterly MDS also showed impaired cognition and dependence or substantial assistance with toileting hygiene, upper body dressing, personal hygiene, lower body dressing, bathing, and showers. During observation, two CNAs entered Resident #54’s room to provide morning hygiene and incontinence care. Although signage on the door indicated enhanced barrier precautions and PPE was available inside the room, the CNAs performed hand hygiene and put on gloves only; they did not don a gown. They completed the care while the resident was rolled toward them and came into direct contact with their uniforms/clothing. Interviews with the CNAs confirmed they did not wear a gown, and one CNA stated they were supposed to wear one because the resident had a feeding tube. The RN/ADON/Infection Control nurse and the Regional Nurse both verified that a gown was required during high contact care for this resident because of the feeding tube. Review of CMS memorandum QSO-24-08-NH and facility policy showed that residents with indwelling devices such as feeding tubes were to receive enhanced barrier precautions during high contact care.
