Facility failed to follow its policy and federal regulations by not posting the Daily Nurse Staffing Posting information in a prominent place readily accessible to residents, staff, and visitors. On 3/2/2026, surveyors observed the main lobby and the 8th floor unit and did not see the Daily Nurse Staffing Posting sheet displayed prominently in either area. The facility had a census of 30 residents living on the 8th and 9th floor units at the time of the survey. On 3/3/2026, surveyors again observed the main lobby, 8th floor unit, and 9th floor unit and did not observe the Daily Nurse Staffing Posting sheets prominently displayed on any bulletin boards or other display areas. An LPN stated she was not aware of any prominently posted sheet containing the daily nurse staffing posting information and said the unit only had the daily schedule at the nurse's station. The DON stated the daily staffing schedules were posted at each nurse's station but was not sure where the Daily Nurse Staffing Posting sheet would be located, and the scheduler stated the sheets were kept in a binder in her office and she did not know they were supposed to be prominently displayed. The administrator later stated the sheets were inside staffing binders at each nurse's station and that the facility would start posting them on each nurse's station bulletin boards.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

The facility policy, Nurse Staffing Posted Information dated 02/01/2026, stated that nurse staffing information was to be made readily available in a readable format to residents, staff, and visitors at any given time.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
