Food Safety, Hand Hygiene, Sanitation, and Storage Deficiencies
Summary
The facility failed to store, prepare, distribute, and serve food in accordance with professional standards in the main kitchen. During meal service, a tray of small souffle cups containing sour cream was left sitting on a metal cart next to the service line without ice. The sour cream remained on the cart while room trays were prepared and sent out to residents, and when the test tray was evaluated later, the sour cream measured 74 degrees F. The assistant dietary manager stated the souffle cups should have been on ice and that dairy products should be kept on ice during meal service. The facility also failed to perform hand hygiene appropriately during meal service and dishwashing. An unidentified dietary staff member prepared a plate without washing or sanitizing hands, wiped her nose with the back of her hand, and then served the plate to a resident. The same staff member later entered and exited the kitchen without washing or sanitizing hands before continuing to serve meals. During meal service, CK #3 handled room trays while wearing the same gloves, touched multiple surfaces including preparation tables, meal trays, and the reach-in refrigerator without changing gloves, and later touched three premade cheeseburgers with the same gloves. At the three-compartment sink, DA #1 moved from dirty dishes to clean dishes without washing his hands. The assistant dietary manager stated staff should change gloves when changing tasks or touching other surfaces and should wash hands when moving from dirty to clean dishes. The kitchen was not kept clean and sanitary, and food was not labeled or stored correctly in multiple storage areas. Observations showed wet towels on preparation tables, a damaged floor and crumbling threshold in the dishroom area, missing cove base, an uncovered trash can, an uncovered mixer with sleeves of styrofoam bowls on top, debris on a plate dolly used for meal service, an overflowing trash can, and damaged flooring and wall areas near the three-compartment sink. Food storage issues included hot dog buns on the floor, frozen vegetables on the freezer floor, unlabeled or undated chopped hard-boiled eggs and pre-poured sauces in the reach-in refrigerator and walk-in refrigerator, personal lunch boxes in the walk-in refrigerator, and dented cans stored with other canned goods. The assistant dietary manager stated boxes of food should never be stored on the floor and that dented cans would normally be removed from use.
