Summary

The facility failed to develop and implement an ongoing program of individualized, meaningful activities for 3 residents (#12, #27, and #67) based on their assessments and care plans. The report states that activities were not consistently offered in accordance with each resident’s identified interests and documented activity goals over extended periods of time, with prolonged gaps in activity offerings and participation. The resident stated that activities had never been offered and that he/she did not recall ever attending one. Resident #67 was admitted with diagnoses including depression and adult failure to thrive, and the care plan identified multiple preferred activities including arts/crafts, education games, cooking/baking, gardening, music, family visits, pet therapy, reading, reminiscing, horse racing, naps, socializing, and beauty breaks. The Activities Flowsheet showed only isolated participation across the year, with many months showing no documented offers or participation. The Activities Director stated that if documentation was not present on the Activities Flowsheet, it meant the resident was not offered an activity, and that tracking residents who had not participated was done by memory.
