Summary

Surveyors identified a deficiency in the facility’s failure to develop and implement a comprehensive, person-centered care plan addressing a resident’s Intravenous Gamma-Globulin (IVGG) therapy for cervical disc disorder at the C5–C6 level with myelopathy. The resident was originally admitted with multiple diagnoses including cervical disc disorder with myelopathy, anemia, hemiplegia, and cerebral infarction, and had decision-making capacity and intact cognitive functioning per the History and Physical and Minimum Data Set. The MDS also documented that the resident required supervision for personal hygiene and substantial assistance for upper and lower body dressing. Physician orders dated 9/16/2025 directed weekly IVGG for the cervical disc disorder with myelopathy, supplied by an outside infusion company and administered by an infusion nurse, and a subsequent order dated 3/26/2026 documented an upcoming in-facility infusion appointment. During an interview and concurrent record review with the Assistant Director of Nursing, it was confirmed that the resident’s care plan did not address the IVGG infusions or the underlying cervical disc disorder with myelopathy. The ADON acknowledged that, despite the IVGG being provided by an external company, it remained the facility’s responsibility to monitor and implement necessary care for the resident and that the care plan is intended to guide staff in monitoring and providing appropriate care. The ADON stated that the failure to address the cervical disease and IVGG infusion in the care plan had the potential to delay care and that there was a potential for the resident to experience an adverse reaction. Review of the facility’s policy on comprehensive, person-centered care plans showed it required development and implementation of a care plan with measurable objectives and timetables to meet each resident’s physical, psychological, and functional needs, including a description of services to attain or maintain the resident’s highest practicable well-being.
