Summary

The deficiency involves the facility’s failure to ensure timely implementation of RD recommendations to maintain a resident’s nutritional status. The resident was admitted and readmitted with diagnoses including DM, dementia, heart failure, and hyperlipidemia, and was on a CCHO, NAS, IDDSI level 6 diet with thin liquids. Weight records showed a progressive decline from 178 lbs on 12/31/2025 to 141 lbs on 3/18/2026. On 3/18/2026, the RD again assessed the resident after a documented 5 lb weight loss in one week and recommended adding LiquaCel twice a day, while continuing fortified diet, magic cup three times a day, Boost twice a day, and the appetite stimulant. The LiquaCel order was not entered until 3/20/2026, two days after the RD’s recommendation. The RD, QA nurse, and DON each acknowledged that RD recommendations should be communicated to and acted upon by the MD within 24 hours (and no later than 72 hours per QA nurse), and that in this case the appetite stimulant and protein supplement orders were delayed beyond those expectations. The facility’s Weight Assessment and Intervention policy indicated that undesirable weight changes are to be evaluated by the treatment team, including consideration of the resident’s nutrient needs compared with intake and the relationship between medical condition and weight fluctuations.
