Summary

The facility failed to maintain the kitchen in a manner that prevents food borne illness, affecting all 135 residents receiving dietary services. During the kitchen tour, the stove had a facility pan with brown gravy covered in plastic wrap over an open flame, two areas of the kitchen were being used to prep food, and only one red sanitation bucket was in use. The red sanitization bucket tested at 0 ppm, and no logs were available for refrigerators, coolers, red buckets, the three-compartment sink, or the dishwasher. The dishwasher was observed dirty inside and out, with the mint-colored flaps caked with a thick tan substance. Test strips used for the dishwasher turned orange at 160 degrees, while the wash dial reached a maximum of 150 degrees Fahrenheit and the rinse dial reached a maximum of 120 degrees Fahrenheit. Staff stated the dish machine temperatures had not been logged in weeks, the sanitizer level of the three-compartment sink had not been logged in over three weeks, and red bucket logs had never been done. The Dietary Manager stated the dishwasher had not reached 180 degrees Fahrenheit since she started working in the facility and said the dishes were still safe to use if the test strip turned orange at 160 degrees Fahrenheit. The mixer, meat slicer, tomato slicer, toaster oven, utensils on an open cart, and two drawers containing utensils were dirty and uncovered. In dry storage, personal winter coats were hanging on storage racks, and multiple food items were unlabeled, undated, or stored with scoops, cups, parchment paper, or garbage-bag liners. Several cans were dented. In the walk-in cooler and freezer, food items were open to air, poorly wrapped, unlabeled, undated, or stored without lids, and the floor had crumbs and an orange spill. The Dietary Manager stated that equipment should be cleaned and covered, personal belongings should not be kept in the kitchen, food items should be labeled and dated, scoops and cups should not be stored in food bins, dented cans should be discarded, and exposed foods should not be left open to air.
